Instructions to the Applicant
The information you provide in this application will be used in the background investigation to assist in determining your suitability for the position of San Andreas Peace Officer, in accordance with POST Commission Regulation 1953.
- It is your responsibility to complete this form and provide all required information.
- This document must be filled out electronically and printed.
- You must respond to all items and questions. If a question does not apply to you, write “D.N.A.†(does not apply) in the space provided for your response.

Disqualification
There are very few
automatic bases for rejection. Even issues of prior misconduct, such as prior illegal drug use, driving under the influence, theft, or even arrest or conviction are usually not, in and of themselves, automatically disqualifying. However,
deliberate misstatements or omissions can and often will result in your application being rejected, regardless of the nature or reason for the misstatements/omissions. In fact, the number one reason individuals “fail†background investigations is because they deliberately withhold or misrepresent job-relevant information from their prospective employer.
BOTTOM LINE: You are responsible for providing complete, accurate, and truthful responses.

- Thomas Cole was born on the night of 17/FEB/1999 to a low-income family in the tough neighborhoods of Davis, the oldest of three brothers.
Being from Davis meant that throughout his entire childhood Thomas was surrounded by gangsters and people with bad influence, Thomas’ mother did not want him influenced by those kinds of people and getting involved in illegal activities and so she did her very best to keep Thomas away from those particular people. Though his mother didn't even need to break a sweat to keep him away from those people since the only thing that ever interested him was basketball, Thomas' love for basketball started when his father used to play with him on the neighborhood's court, Thomas would stay out all night playing on the court by himself, and as they say "practice makes perfect" and so it did, Thomas became good at basketball, but as you already know, life isn't always pink and sweet, Thomas would lose games from time to time as well, which made him upset since he's a winner at heart, but every time he lost a game his mother would encourage him and tell him the same sentence - "Dream big, start small but start now" and that's what kept him motivated to carry on and practice even harder.
A few years has passed by and Thomas enrolled into Davis High, as soon as he could he started looking for opportunities to join the basketball team, after going through the tryouts for the basketball team, he managed to get in as a point guard, his favorite position, Thomas was an excellent player, you'd always find him putting in extra hours of practicing his shots and looking to better his plays and skills, he was undoubtedly a hard-worker when it came to the basketball court;
Though when it came to his school grades, the situation wasn't the same, his grades weren't the best, which didn't allow him to get a college scholarship and his parents couldn't afford to pay for his college so he had to stop doing the only thing he absolutely loved, playing basketball.
A few months had passed by and he started looking for a job, that's when he found out about IMEX, he knew he had to make money somehow to pay for college because he couldn't give up on his dream, he managed to get himself a job at IMEX as a truck driver, a couple of months of him trucking had passed by and he heard about an opening in the HR department, he asked the CEO for it and went into an interview for the HR position, after a few days of patiently waiting for an answer, the CEO returned to Thomas with a positive answer and at this point, he had already forgotten about college since it was already too late, he meanwhile became a Senior HR and a client's agent for IMEX.
Even though he enjoyed driving trucks, interviewing people, and getting clients for IMEX, Thomas has always been a hyperactive person who highly enjoys being on the move and being useful, hence, why he started looking for other career paths that would allow him to develop his skills further, that's when he heard about LSSD and their various units with more opportunities that he can get if he joined. He knew it would take hard work and dedication to join the LSSD, but as his mother always used to tell him "Dream big, start small but start now" and so here he is, dreaming big, starting small but starting now.
